STEMI and, in particular, the resulting left ventricular remodelling represent one of the main factors influencing long-term prognosis. However, left ventricular remodelling can differ considerably even in patients with similar clinical, angiographic, and therapeutic features, and the reasons for this variability are still not well understood. Moreover, inflammation has emerged as a key pathophysiological driver of myocardial injury and repair after STEMI, but its specific role in determining the trajectory of left ventricular remodelling is still unclear. Lots of clinical, biochemical, and imaging predictors of left ventricular remodelling have already been identified, but it is still unclear how systemic inflammation, standard biomarkers, and advanced cardiac magnetic resonance parameters interact with each other in predicting post-infarction remodelling outcomes. Furthermore, whether these inflammatory mechanisms extend beyond obstructive STEMI to myocardial infarction with non-obstructive coronary arteries (MINOCA) presenting with ST-segment elevation remains largely unexplored.
The aim of this study is twofold: first, to confirm the existence of a shared inflammatory substrate between STEMI and MINOCA, through an integrated analysis of systemic inflammatory biomarkers and epicardial adipose tissue thickness; second, to evaluate the role of this inflammatory profile in predicting adverse and reverse ventricular remodelling after STEMI, in comparison with multiparametric cardiac magnetic resonance parameters reflecting myocardial injury and microvascular damage
