Riassunto
The purpose of this thesis was to demonstrate, after a careful analysis, the feasibility of cutting metal implants (in particular the OTSC clip produced by Ovesco AG, Tuebingen, Germany) using high density DC current and extracting them from the GI tract by the help of a 3.2 mm working channel endoscope. The first step was to compare this physical principle with others relevant to this purpose (see APC or Nd:YAG laser). Then it was tested thoroughly in ex-vivo experimental trials, in-vivo on animal trials and in clinical uses obtaining satisfactory results. The principle consists of applying a pulse of about 100 ms, 30 V and 100 A on a metal side by the use of an instrument manufactured in such dimensions that it can be inserted into the working channel of the endoscope. The purpose set, was successfully fulfilled, as far as the cutting of the OTSC clip is concerned, but indicated also the feasibility of cutting other metal implants such as the stents.
