Background: Eosinophilic esophagitis (EoE) diagnosis and monitoring still rely on invasive esophagogastroduodenoscopy (EGD) with biopsies. Non-invasive biomarkers are therefore needed. This study prospectively assessed the diagnostic and follow-up value of serum Eotaxin-1, Eotaxin-2, Eotaxin-3, IL-4, IL-5, and IL-10 in adult EoE patients.
Methods: Adults (≥18 years) with new EoE (≥15 eosinophils/HPF) were enrolled. At baseline (T0), all underwent EGD with six biopsies. After standard therapy, histological remission was reassessed at 12 weeks (T1). Serum cytokine levels were measured at T0 and T1 and compared with age-, sex-, and atopy-matched healthy controls (HCs).
Results: Twenty-five EoE patients and fourteen HCs were included. At T0, EoE patients had higher Eotaxin-3 (501 vs. 224 pg/mL, p=0.002) and IL-10 (100 vs. 56 pg/mL, p=0.01). Eotaxin-3 ≥389 pg/mL (AUC=0.80; 93% specificity; 68% sensitivity) and IL-10 ≥168 pg/mL (AUC=0.74; 100% specificity; 44% sensitivity) identified active EoE. After treatment, 18/25 (72%) achieved remission, showing lower Eotaxin-3 (252 vs. 501 pg/mL, p=0.02) and IL-10 (67 vs. 100 pg/mL, p<0.05). Remission cut-offs were 241 pg/mL for Eotaxin-3 (AUC=0.70) and 199.5 pg/mL for IL-10 (AUC=0.68). Conclusions: Serum Eotaxin-3 and IL-10 are promising non-invasive biomarkers for diagnosing and monitoring eosinophilic esophagitis.
