Gamma oscillations support complex cognitive processes, including neuroplasticity, working memory, and inter-areal functional integration. Their dysregulation has been implicated in several pathological conditions, including stroke, making gamma rhythm a promising therapeutic target.
In this context, Gamma Entrainment Using Sensory Stimulation (GENUS) has emerged as a non-invasive approach aimed at synchronizing brain activity to a predetermined frequency through sensory stimulation. In particular, 40 Hz stimulation has been shown to effectively induce oscillations within the physiological gamma band.
The present two-phase study aims to promote neuroplasticity through 40 Hz visual stimulation, thereby potentially enhancing post-stroke rehabilitation.
In the first phase, a pilot study conducted in healthy participants confirmed the effectiveness of the specifically developed experimental setup in inducing entrainment, with propagation extending beyond the occipital cortex toward more anterior brain regions. A cognitive-motor Go/No-Go protocol was then designed to compare performance following training either associated or not associated with 40 Hz stimulation.
After safety and feasibility had been assessed in healthy participants, the protocol was preliminarily translated to post-stroke patients, confirming the induction of the effect during exposure to the 40 Hz stimulus in this population as well. The extension of the task-based protocol to this population was therefore proposed, with the aim of evaluating its tolerability, feasibility, and effects on performance.
This ongoing study may contribute to defining 40 Hz visual GENUS as a non-invasive strategy that can be integrated into post-stroke neurorehabilitation approaches, with potential future applications in other pathological conditions.

Le oscillazioni gamma sostengono processi cognitivi complessi quali neuroplasticità, memoria di lavoro ed integrazione funzionale inter-area. Una loro disregolazione è implicata in numerose patologie, incluso l’ictus, configurando il ritmo gamma come promettente bersaglio terapeutico.
In questo contesto si inserisce la Gamma Entrainment Using Sensory Stimulation (GENUS), metodica che sincronizza l’attività cerebrale ad una frequenza prestabilita dalla stimolazione sensoriale. L’impiego di uno stimolo a 40 Hz, in particolare, è risultato efficace nell’indurre oscillazioni nella banda gamma fisiologica.
Il presente studio, articolato in due fasi, mira a promuovere la neuroplasticità attraverso stimolazione visiva a 40 Hz e potenziare così la riabilitazione post-ictus.
Nella prima fase, uno studio pilota su soggetti sani ha confermato l’efficacia del setting appositamente sviluppato nel generare entrainment, con una propagazione oltre la corteccia occipitale verso aree più anteriori. È stato quindi elaborato un protocollo cognitivo-motorio Go/No Go, volto a confrontare le prestazioni dopo un training associato o meno alla stimolazione a 40 Hz. Valutate sicurezza e fattibilità sui soggetti sani, il protocollo è stato preliminarmente traslato ai pazienti post-ictus, confermando anche in questi ultimi l’induzione dell’effetto durante l’osservazione dello stimolo a 40 Hz. È stata quindi proposta l’estensione del protocollo con task anche in tale popolazione, con l’obiettivo di valutarne tollerabilità, fattibilità ed effetto sulle prestazioni. Lo studio, attualmente in corso, contribuirà a definire la GENUS visiva a 40 Hz come strategia non invasiva integrabile agli approcci neuroriabilitativi post-ictus, con possibili prospettive future anche in ulteriori condizioni patologiche.
