The present work tries to answer the question of if there exist biomarkers predictive of sensitivity or resistance to immune checkpoint inhibitors in metastatic colorectal cancer. We performed transcriptomic analyses on a cohort of 110 Italian patients and applied two literature-published signatures before comparing RNAseq results with clinical data. We identified a transcriptional profile that is expressed more significantly in patients with short progression-free survivals. These data prove the starting hypothesis but will need further validation to confirm the potential of transcriptomic analysis as a predictive biomarker of response/resistance to ICIs in mCRC.
