Obiettivi: sviluppare un modello predittivo di NTCP per RILD grave su uno studio prospettico monocentrico introducendo SWOARs nell'analisi del DVH e usando l'approccio machine learning.
Metodi: sono stati arruolati candidati per tumori del rino e orofaringe (stadio II-VA) per la radiochemioterapia che richiedono irradiazione bilaterale del collo. È stata eseguita una valutazione della disfagia, inclusa la videofluoroscopia, per valutare il punteggio di aspirazione della penetrazione (P/A-VF) al basale e a 6 e 12 mesi (m) dopo il trattamento. Tutti i piani di RT sono stati ottimizzati per ridurre la dose agli SWOARs. Sono stati sviluppati modelli per predire la comparsa di RILD analizzando la dose a 9 SWOARs (muscolo costrittore superiore, medio e inferiore, SPCM; MPCM; IPCM; base della lingua, BOT; laringe sopra e glottica, SL e GL; ghiandole parotidi, PGs; esofago cervicale, EC ) e i dati di P/A-VFS. Sono state estratte in totale 72 features per ogni paziente (8 dati per SWOAR: Dmin, Dmax, Dpeak, Dmean, V35, V45, V55, V65) e analizzate con Support Vector Machine lineare (SVM) e Bagged Tree Classification (BTC). Le prestazioni sono state valutate calcolando l'AUC della curva ROC. Le caratteristiche sono state classificate per importanza utilizzando stime dell'importanza predittiva out-of-bag per permutazione.
Risultati: la capacità predittiva di RILD attraverso la valutazione delle features ricavate dal DVH delle SWOARs è emersa sia a 6 che a 12 m: AUC 0,82 con SVM e AUC 0,83 con BTC (6m); AUC 0,86 con SVM e 0,93 con BTC (12m). Le SWOARs e le features con la massima importanza nel predire RILD a 6 m sono BOT (V65 e Dmean), SPCM (Dmean), MPCM (V45, V55; V65; Dpeak; Dmean; Dmax e Dmin) e PGs (Dmean e Dpeak) mentre quelle più significative a 12 m sono MPCM (V55; Dmax e Dmean), IPCM (V55, V65 Dmin e Dmax) insieme a GL (V55 e Dmin), CPM (Dmin) e EC ( dmin).
Conclusioni: abbiamo ottenuto un modello NTCP di RILD con elevata capacità discriminatoria sia a 6 che a 12 m dopo RTCT. Lo studio multicentrico NCT 03448341 sarà importante per validare e allenare ulteriormente il sistema.

Aims: To develop a predictive NTCP model for severe RILD upon a prospective monocentric study by introducing SWOARs in the DVH analysis and using machine learning approach.
Methods: Naso and oropharynx cancers (Stage IIIVA)candidates for radiochemotherapy requiring bilateral neck irradiation were enrolled. A dysphagia assessment including Videofluoroscopy was performed to assess the Penetration Aspiration score (P/A-VF) at baseline and at 6 and 12 months (m) after treatment. All RT plans were optimized to reduce dose to the SWOARs. Models of severe RILD were developed using dose to 9 SWOARs (superior, medium and inferior constrictor muscle, SPCM; MPCM; IPCM; base of tongue, BOT; supra and glottic larynx, SL and GL; parotid glands, PGs; cricopharyngeal muscle, CPM; cervical esophagus, EC) and P/A-VF data at 6 and 12 m after treatment. A total of 72 features for each patient were extracted (8 features for each SWOAR: Dmin,Dmax,Dpeak,Dmean,V35,V45,V55,V65) and analyzed with linear Support Vector Machine (SVM)and Bagged Tree Classification (BTC). Performances were evaluated computing the Area (AUC) under theReceiver Operating Characteristic (ROC) curve, estimated in leave-one-out cross validation. Features were ranked for importance using out-of-bag predictor importance estimates by permutation.
Results: The relevance of SWOARs DVH features in predicting severe RILD emerged both at 6 and 12 m: AUC 0,82 with SVM and AUC 0,83 with BTC (6m); AUC 0,86 with SVM and 0,93 with BTC (12m). The SWOARs and the correspondent features with the highest importance to avoid severe RILD at 6 m resulted BOT (V65 and Dmean), SPCM (Dmean), MPCM (V45,V55;V65;Dpeak;Dmean;Dmax and Dmin), and PGs (Dmean and Dpeak) whereas those with the highest importance at 12 m were MPCM (V55; Dmax and Dmean), IPCM (V55, V65 Dmin and Dmax) together with GL (V55 and Dmin), CPM (Dmin) and EC (Dmin).
Conclusions: We have trained and cross validated a NTCP model of severe RILD with a high discriminative ability both at 6 and 12 m after RTCT. We expect to improve the generalization capability of this model enlarging the number of training data with the ongoing prospective Italian Multicentric Study (NCT 03448341) in order to be suitable for clinical and planning decision-support.
