OSTEONECROSI DELLE OSSA MASCELLARI DA MEDICAMENTO (MRONJ) ASSOCIATA A PERIMPLANTITE: STUDIO RETROSPETTIVO DI 32 CASI

Scopo: l’osteonecrosi delle ossa mascellari da farmaci (MRONJ) rappresenta un effetto avverso conseguente al trattamento con farmaci antiriassorbitivi e farmaci antiangiogenetici. Mentre in letteratura è riportata una stretta associazione tra l’estrazione di un elemento dentario e lo sviluppo MRONJ, i dati sono controversi riguardo il posizionamento implantare. Da un punto di vista clinico, possiamo descrivere due scenari di rischio: il primo riguarda il rischio di MRONJ associato al posizionamento implantare ed il secondo riguarda il rischio di MRONJ associato a perimplantite. Lo scopo di questo studio è quello di presentare le caratteristiche cliniche, radiografiche ed il trattamento chirurgico di una coorte di pazienti affetti da osteonecrosi dei mascellari associata a perimplantite.
Metodi: al fine di valutare l’ipotesi dello sperimentatore è stato disegnato uno studio retrospettivo. Sono stati inclusi tutti i pazienti a cui era stata diagnosticata una MRONJ associata a perimplantite trattati presso il reparto di Odontostomatologia e Chirurgia Orale dell’Ospedale Universitario di Pisa da gennaio 2013 a dicembre 2020. La diagnosi di MRONJ è stata formulata sulla base dell’integrazione del dato clinico e radiografico sulla base della definizione della American Association of Oral and Maxillofacial Surgeons (AAOMS) del 2014. Sono stati raccolti dati riguardanti anamnesi familiare e medica di ciascun paziente, tipologia e durata del trattamento antiriassorbitivo e caratteristiche delle lesioni osteonecrotiche.
Risultati: sono stati inclusi nello studio trentadue pazienti (27 femmine; età media: 65,74 anni) che presentavano 59 impianti associati a MRONJ. Diciotto pazienti erano in trattamento con bifosfonati per via endovenosa (acido zoledronico, 4 mg/mese) per il trattamento di patologie oncologiche: metastasi da carcinoma mammario (9 soggetti, 28.1%), mieloma multiplo (6 soggetti, 18.7%), metastasi da carcinoma polmonare (2 soggetti, 6.4%). Quattordici pazienti assumevano bifosfonati per via orale per il trattamento dell’osteoporosi. Undici pazienti erano fumatori, 4 affetti da diabete, 9 affetti da ipertensione arteriosa. Lo stadio più frequente di MRONJ è risultato essere lo stadio III (21 soggetti, 66%), seguito dallo stadio II (11 soggetti, 34%). In tutti i casi le lesioni erano sintomatiche ed in meno della metà dei casi si evidenziava esposizione di tessuto osseo (15 casi, 47%). Le lesioni erano prevalentemente localizzate a livello mandibolare (22 lesioni, 68.7%). Al follow-up di 12 mesi, si evidenzia un tasso di guarigione completa nell’84,4% dei casi, con il 100% di successo per le osteonecrosi localizzate a livello del mascellare. I cinque casi resistenti al trattamento chirurgico erano localizzati a livello mandibolare. Stratificando il tasso di successo in base allo stadio della lesione si evidenzia il 100% di guarigione per le lesioni in stadio II e dell’83% per le lesioni in stadio III.
Conclusioni: l’osteonecrosi dei mascellari associata a perimplantite rappresenta una realtà clinica emergente. Tutti i pazienti portatori di impianti dentali che si apprestano ad iniziare una terapia con farmaci antiriassorbitivi e/o antiangiogenetici dovrebbero essere adeguatamente informati del maggiore rischio di sviluppare lesioni intorno agli impianti stessi. La rimozione degli impianti associata a toilette chirurgica rappresenta una valida opzione terapeutica per il trattamento di questo quadro clinico.
OSTEONECROSIS OF THE MAXILLARY MEDICATION BONES (MRONJ) ASSOCIATED WITH PERIMPLANTITIS: RETROSPECTIVE STUDY OF 32 CASES

Purpose: drug-induced osteonecrosis of the jaw bones (MRONJ) represents an adverse effect following treatment with antiresorptive drugs and antiangiogenic drugs. While a close association between the extraction of a dental element and MRONJ development is reported in the literature, the data are controversial regarding implant placement. From a clinical point of view, we can describe two risk scenarios: the first concerns the risk of MRONJ associated with implant placement and the second concerns the risk of MRONJ associated with peri-implantitis. The aim of this study is to present the clinical, radiographic and surgical treatment features of a cohort of patients affected by osteonecrosis of the jaw associated with peri-implantitis.
Methods: in order to evaluate the investigator's hypothesis, a retrospective study was designed. All patients diagnosed with MRONJ associated with peri-implantitis treated at the Department of Odontostomatology and Oral Surgery of the University Hospital of Pisa from January 2012 to December 2020 were included. The diagnosis of MRONJ was made on the basis of integration clinical and radiographic data based on the definition of the American Association of Oral and Maxillofacial Surgeons (AAOMS) of 2014. Data were collected regarding the family and medical history of each patient, type and duration of antiresorptive treatment and characteristics of osteonecrotic lesions.
Results: Thirty-two patients (27 females; mean age: 65.74 years) who had 59 MRONJ associated implants were included in the study. Eighteen patients were being treated with intravenous bisphosphonates (zoledronic acid, 4 mg / month) for the treatment of oncological diseases: breast cancer metastases (9 subjects, 28.1%), multiple myeloma (6 subjects, 18.7%), metastases from lung cancer (2 subjects, 6.4%). Fourteen patients took bisphosphonates orally for the treatment of osteoporosis. Eleven patients were smokers, 4 with diabetes, 9 with arterial hypertension. The most frequent stage of MRONJ was found to be stage III (21 subjects, 66%), followed by stage II (11 subjects, 34%). In all cases the lesions were symptomatic and in less than half of the cases there was exposure of bone tissue (15 cases, 47%). The lesions were predominantly localized in the mandibular area (22 lesions, 68.7%). At the 12-month follow-up, there was a complete cure rate in 84.4% of cases, with 100% success for osteonecrosis localized in the jaw. The five cases resistant to surgical treatment were located at the mandibular level. Stratifying the success rate based on the stage of the lesion shows 100% healing for stage II lesions and 83% for stage III lesions.
Conclusions: osteonecrosis of the jaws associated with peri-implantitis represents an emerging clinical reality. All patients with dental implants who are preparing to start therapy with antiresorptive drugs should be adequately informed of the increased risk of developing lesions around the implants themselves. The removal of the implants associated with a surgical toilet represents a valid therapeutic option for the treatment of this clinical picture.
