Riassunto
L’elettrochemioterapia è il risultato della combinazione di due effetti: l’elettroporazione delle membrane cellulari e la somministrazione di farmaci chemioterapici. L’elettroporazione è una tecnica innovativa che si basa sull’applicazione locale di impulsi elettrici brevi ed intensi, che permeabilizzano reversibilmente le membrane cellulari al fine di facilitare il trasporto degli agenti chemioterapici nel citoplasma, aumentandone la propria intrinseca efficacia antitumorale e permettendo un effetto terapeutico efficace a dosi così basse da non determinare tossicità sistemica, senza colpire i tessuti non esposti agli impulsi. In questo studio sono stati arruolati 12 casi clinici di cane e 3 di cavallo che presentavano neoplasie della cute e/o sottocute o delle mucose, sia di natura benigna che maligna. I pazienti sono stati sottoposti a diversi protocolli di elettrochemioterapia, al fine di testare l’efficacia di questa tecnica, utilizzata singolarmente per la riduzione della massa tumorale oppure in modo sinergico alla chirurgia o successivamente ad essa. È stata monitorata l’insorgenza di recidive post-trattamento al fine di valutarne le cause. Dallo studio è emerso che il 63,6% dei cani trattati ha avuto una remissione completa senza recidiva, il 9% ha avuto una remissione completa con successiva recidiva, il 18% ha presentato remissione temporanea e successiva recidiva e il 9% nessuna remissione. Il 50% dei cavalli trattati ha avuto una remissione completa senza recidiva, e l’altro 50% ha avuto remissione temporanea con successiva recidiva. Dallo studio si è potuto dimostrare che l’ECT è una terapia applicabile ed efficace e che ha diversi vantaggi rispetto ad altre tecniche anti-tumorali perciò presenta un ottimo potenziale di utilizzo nel futuro della medicina oncologica veterinaria.
Electrochemotherapy is the result of the combination of two effect: the electroporation of cell membranes and the administration of chemotherapeutic drugs. Electroporation is an innovative technique which is based on the local application of short and intense electrical pulses, that determines the reversible permeabilization of cell membranes in order to facilitate the transport of the chemotherapeutic agents into the cytoplasm, thus increasing its intrinsic antitumor efficacy and allowing a therapeutic effect effective at low doses that do not cause systemic toxicity, without affecting the tissues not exposed to pulses. In this study 12 of dogs and 3 of horses were enrolled presententing skin and/or subcutaneous tissue or mucous membranes tumours, both benign and malignant. Patients were subjected to different protocols electrochemotherapy, in order to test the effectiveness of this technique, which is used individually for the reduction of tumour mass or in a synergistic way to surgery or any time thereafter. It was monitored the onset onset of recurrences post-treatment in order to assess the causes. The study found that 63.6% of treated dogs had a complete remission without recurrence, 9% had a complete remission with subsequent relapse, 18% had temporary remission and subsequent relapse and 9% hadn’t remission. 50% of the horses treated had a complete remission without recurrence, and 50% had temporary remission followed by relapse. From the study it was demonstrated that ECT is a treatment applicable and effective and it has several advantages over other anti-tumour techniques. Therefore, ECT has a great potential for use in the future of medical oncology in the field of veterinary medicine.
