Riassunto
Obiettivo – Lo scopo del lavoro è stato quello di valutare l’applicabilità e l’efficacia dell’utilizzo dell’ultrasonografia nel monitoraggio degli accessi vascolari periferici in cani ricoverati.

Materiali e Metodi – Lo studio è stato svolto su un campione di 20 cani ricoverati nel reparto di terapia intensiva dell’ospedale didattico veterinario “Mario Modenato” dell’università di Pisa. I pazienti sono stati sottoposti ad una valutazione ecografica e clinica seriale mediante score ogni 24 ore a partire dal posizionamento dell’accesso vascolare. La valutazione ecografica è stata eseguita con un ecografo portatile utilizzando una sonda lineare con frequenza da 8 a 13 MHz. La valutazione è stata eseguita mediante l’attribuzione di punteggi definiti due score multiparametrici, di cui uno ecografico e uno clinico.

Risultati – Nel campione in studio, per 16 pazienti il catetere è stato rimosso dopo 24 ore, mentre per 4 pazienti il catetere è stato rimosso dopo 48 ore, pertanto su tutti i pazienti è stata eseguita la valutazione a T24, mentre per 4 pazienti è stata eseguita anche a T48. Il punteggio medio di partenza per la valutazione ecografica è di 14,05 punti su 15, mentre a T24 è di 9,80 punti su 15 e a T48 è di 8,25 punti su 15. Tra T0 e T24 il punteggio medio si è abbassato del 30,24%, tra T24 e T48 il punteggio medio si è abbassato del 15,82 %, mentre tra T0 e T48 il punteggio medio si è abbassato del 41,28%.
Il punteggio medio ottenuto dalla valutazione clinica al momento del posizionamento del catetere vascolare è stato di 11,9 punti su 12, mentre dopo 24 ore è stato di 9,8 punti su 12 e dopo 48 ore è stato di 10, 25 punti su 12. In percentuale, il punteggio medio tra T0 e T24 si è ridotto del 17,65%, mentre tra T0 e T48 si è ridotto del 13,87%. Il punteggio medio tra T24 e T48 ha subìto un aumento del 4,59%.

Conclusioni – lo studio ha dimostrato una netta discordanza tra i punteggi ottenuti dalla valutazione ecografica e quelli ottenuti dalla valutazione clinica, in particolare lo score ecografico è risultato maggiormente sensibile nell’identificare precocemente le alterazioni tipiche delle vene cateterizzate.
L’ecografia può essere quindi considerabile uno strumento diagnostico utile ad inquadrare meglio le complicazioni relative all’accesso vascolare al fine di elaborare una prognosi più precisa e ponderare la scelta di sostituire o meno il catetere nei momenti di incertezza.

Objective – The aim of this work was to evaluate the applicability and effectiveness of ultrasonography in the monitoring of peripheral vascular accesses in hospitalized dogs.

Material and Methods – this study was carried out on a sample of 20 dogs hospitalized in the intensive care unit of “Mario Modenato” veterinary teaching hospital of the University of Pisa. Patients underwent to serial ultrasound and clinical evaluation every 24 hours starting from the positioning of the vascular access. The ultrasound evaluation was performed with a portable ultrasound system, using a linear probe with a range of frequency of 8 to 13 MHz. The evaluation was performed by assigning two multiparametric scores, one of which was ultrasound and one clinical.

Results – In the study sample, for 16 patients the catheter was removed after 24 hours, while for 4 patients the catheter was removed after 48 hours, therefore on all patients the evaluation at T24 was performed, while for 4 patients it was performed also at T48. The average starting score for the ultrasound evaluation is 14.05 points out of 15, while at T24 it is 9.80 points out of 15 and at T48 it is 8.25 points out of 15. Between T0 and T24 the average score dropped by 30.24%, between T24 and T48 the average score dropped by 15.82%, while between T0 and T48 the average score fell by 41.28%.
The mean score obtained from the clinical evaluation at the time of vascular catheter placement was 11.9 points out of 12, while after 24 hours it was 9.8 points out of 12 and after 48 hours it was 10.25 points out of 12. As a percentage, the average score between T0 and T24 decreased by 17.65%, while between T0 and T48 it decreased by 13.87%. The mean score between T24 and T48 increased by 4.59%.

Conclusion – this study showed a clear discrepancy between the scores obtained from the ultrasound evaluation and those obtained from the clinical evaluation, in particular the ultrasound score was more sensitive in identifying the typical alterations of the catheterized veins early on.
Ultrasound can therefore be considered a useful diagnostic tool to better frame the complications relating to vascular access in order to develop a more precise prognosis and weigh the choice of whether or not to replace the catheter in moments of uncertainty.
