Introduzione: Le fratture periprotesiche dopo protesizzazione d’anca rappresentano un problema clinico in costante aumento. Fra queste, le fratture di tipo B rappresentano una complicanza complessa da diagnosticare in modo corretto e da trattare chirurgicamente. Lo scopo di questo studio di tipo retrospettivo è stato quello di rivalutare il tipo di trattamento chirurgico delle fratture periprotesiche post-operatorie tardive di tipo B effettuato presso il nostro Istituto, il tasso di complicanze associato e l’outcome clinico dei pazienti trattati.

Materiali e metodi: Abbiamo esaminato i casi trattati nel periodo compreso fra il gennaio 2012 e il febbraio 2018, classificandoli secondo il sistema U.C.S. AO/OTA. Abbiamo valutato l’outcome radiografico secondo i criteri di Beals and Tower. Sono stati inoltre rivalutati clinicamente i pazienti ancora in vita secondo l’H.H.S. e il WOMAC score.

Risultati: In questo periodo sono stati trattati 48 pazienti (12 uomini, 35 donne, età media 81 anni), suddivisi in 24 fratture di tipo B1, 14 di tipo B2 e 10 di tipo B3. Il tasso di consolidazione è stato complessivamente del 95,4% mentre quello di complicanze maggiori (lussazione dell’impianto, pseudoartrosi, infezione profonda) è stato del 12,5%. Clinicamente è stato è possibile rivalutare 34 pazienti con un follow-up medio di 38,4 mesi, un HHS medio di 75,89 e un WOMAC score medio di 79,93.

Conclusioni: Le fratture periprotesiche di tipo B sono difficili da gestire, necessitano di un’accurata pianificazione preoperatoria e un’appropriata gestione intraoperatoria. Il risultato clinico e radiografico complessivo è stato comunque soddisfacente, anche se i pazienti devono comunque essere messi al corrente del rischio di complicanze associate a questo tipo di frattura.

Introduction: Periprosthetic fractures after hip prosthesis represent a constantly increasing clinical problem and a challenging complication to treat surgically. Among these type B proximal femur fractures should be diagnosed correctly to be treated surgically. The aim of this study was to re-evaluate the type of surgical treatment of periprosthetic fractures.

Materials and methods: We examined the cases treated between January 2012 and February 2018, classifying them according to the U.C.S. AO / OTA. We evaluated the radiographic outcome according to the Beals and Tower criteria. Patients still alive were also re-evaluated according to the H.H.S. and the WOMAC score.

Results: We treated 48 patients (12 men, 35 women, average age 81 years), divided into 24 type B1, 14 type B2 and 10 type B3 fractures. The overall consolidation rate was 95.4%, while the major complications (implant dislocation, pseudoarthrosis, deep infection) rate was 12.5%. Clinically it was possible to reassess 34 patients with a mean follow-up of 38.4 months, an average HHS of 75.89 and a mean WOMAC score of 79.93.

Conclusions: Periprosthetic type B fractures are difficult to manage, require careful preoperative planning and appropriate intraoperative management. However, the overall clinical and radiographic result was satisfactory, although patients should still be aware of the risk of complications associated with this type of fracture.
