Trichinellosis is a zoonotic parasitic infection caused by a nematode of the genus Trichinella, with a worldwide distribution. In this genre there are nine recognized species and two clades: one encapsulated and one non-encapsulated. The cellular immune response to Trichinella is well known in the rodent, in which after a short Th1 response a Th2 polarization follows which is maintained during the whole period of the infection. However, the knowledge in human infection is poor. <br>It has been tested 31 samples of subjects recruited from an outbreak occurred in Garfagnana in 2012.<br>The serodiagnosis was carried out with three different commercial ELISA kits and compared with the reference test, ELISA tests in house, developed and validated by the Istituto Superiore di Sanita (ISS). The above serologic tests have been compared with the reference test and the overall acceptance and Cohen’s value has been evaluated.<br>It was also carried out a study of the cellular immune response in the muscle of a patient being infected together with other 51 people in an outbreak of trichinosis in which it was not possible to identify the Trichinella species involved. The blood sample was collected after 10 months of infection.<br>Were isolated from peripheral blood mononuclear cells (PBMC) and incubated with antigens secreted / excreted, (E/S), two species Trichinella : T. spiralis ( T1) and T.pseudospiralis (T4) to determine the index of proliferation. Other PBMCs has been used to produce T antigen specific clones whose phenotype (T helper or cytotoxic cells) and cytokine profile (IFN - ɤ , IL- 17 , IL- 4) were analysed respectively by cytometry flow and ELISA. Through the application of cellular immunology techniques, it has been possible to identify the species of Trichinella infection responsible; confirming the results previously obtained by serological analysis. Based on the results obtained, it is possible to conclude that the causative species was T.pseudospiralis. All results show that in the course of human trichinosis is a mixed Th1 and Th2 response. For the first time was revealed in a chronic infection of human Th17 cells.<br>
