INTRODUZIONE: La terapia antalgica, detta anche terapia del dolore o medicina del dolore consiste nell’approccio terapeutico e scientifico al trattamento del dolore. Si può parlare di trattamento con mezzi farmacologici o non farmacologici. Il trattamento con mezzi farmacologici si basa principalmente su analgesici non oppiacei, oppiacei, antidepressivi triciclici, anticonvulsivanti. La morfina è l’alcaloide più attivo e maggiormente presente nell’oppio (10%). Essa rappresenta l’oppioide meglio conosciuto e ciò è confermato anche dal fatto che la potenza antidolorifica dei farmaci analgesici viene rapportata quasi sempre a quella della morfina. Ancora oggi, la morfina è considerata il farmaco analgesico di prima scelta nel dolore molto intenso, come nel cancro, nel dolore post-operatorio e nel dolore neuropatico. Nel dolore sia acuto che cronico, l’attuale terapia farmacologica ha delle limitazioni considerevoli dovute alla parziale efficacia dei farmaci antidolorifici o ai notevoli effetti collaterali conseguenti all’uso di tali farmaci. Inoltre, esistono anche pazienti che non rispondono più al trattamento farmacologico. Tali limitazioni sono particolarmente evidenti in patologie severe come il trattamento farmacologico del dolore nel cancro o del dolore post-operatorio. Esiste anche una notevole variabilità inter-individuale nella percezione stessa del dolore legata a fattori genetici, ambientali e socio-culturali. Date tutte queste problematiche, tutt’oggi continua la ricerca di nuove terapie che limitino le complicazioni mantenendo la qualità terapeutica. A questo proposito, negli ultimi anni molti ricercatori hanno focalizzato il loro interesse sul possibile ruolo di SIRT 1 nel dolore, evidenziando come non solo SIRT 1, ma anche AMPK e mTOR (due proteine strettamente correlate a SIRT 1) potrebbero essere coinvolte nella modulazione del dolore neuropatico e che un incremento dell’attività di SIRT 1 potrebbe essere una strategia terapeutica promettente per la prevenzione o il trattamento del dolore neuropatico. In aggiunta, studi recenti ipotizzano un possibile coinvolgimento di SIRT 1 nella tolleranza cronica alla morfina. L’importanza di SIRT 1 nel dolore potrebbe essere amplificata proprio dalla sua azione inibitoria su mTOR, descritto in molti lavori come la causa principale dell’assuefazione da morfina e dell’ipersensibilità al dolore.
SCOPO: Nonostante la presenza di molti studi che ipotizzano un ruolo importante di SIRT 1 nel dolore e nella terapia del dolore, sono necessari altri studi volti a confermare questi dati o a chiarire i target specifici con cui SIRT 1 interagisce in questo contesto. Sulla base di queste premesse, questo progetto sperimentale si pone come scopo generale la valutazione dell’effetto di sostanze attivatrici di SIRT 1 su modelli di dolore in vitro. In particolare, verrà valutato l’effetto sia di morfina sia di attivatori di SIRT 1, in seguito a somministrazione singola o associata.
MATERIALI E METODI: Le linee cellulari utilizzate sono neuroni umani di neuroblastoma, SH-SY5Y. Le cellule sono state trattate con Morfina (1µM; 5µM) e due attivatori di SIRT 1, Acido Ferulico (25µM; 50µM) e Tirosolo (10µM; 20µM), utilizzati sia
singolarmente che in associazione. I tempi di incubazione sono stati di 3 e 6 h. I livelli di espressione di SIRT 1 e l’attivazione/inibizione di AMPK e mTOR sono stati valutati mediante la tecnica del Western Blot.
RISULTATI: I risultati dei nostri esperimenti evidenziano come l’espressione di SIRT 1 sia significativamente aumentata in tutti i gruppi sperimentali rispetto al gruppo controllo, sia quando le cellule con trattate con morfina sia quando sono trattate con tirosolo ed acido ferulico. L’analisi dei risultati delle associazioni tra le sostanze (attivatori di SIRT 1 + morfina) ha messo in evidenza come la loro somministrazione associata comporti un aumento significativo dell’espressione di SIRT 1 rispetto a quello ottenuto sommando gli effetti delle singole somministrazioni. In aggiunta, tutte e tre le sostanze determinano un’attivazione di AMPK e un’inibizione di mTOR quando somministrate singolarmente.
CONCLUSIONI: I risultati del presente lavoro di tesi evidenziano che tutte le sostanze testate non solo sono in grado di attivare SIRT 1, ma sono in grado anche di attivare AMPK ed inibire mTOR. L’importanza di questi dati è avvalorata dal ruolo di queste proteine nel pathway del dolore, confermando, infine, il ruolo di SIRT 1 come struttura critica nel pathway nocicettivo e l’importanza di un incremento dell’attività di SIRT 1, come promettente strategia terapeutica per la prevenzione o il trattamento del dolore acuto o persistente.

INTRODUCTION: The antalgic therapy, also known as pain therapy or pain medicine is the therapeutic and scientific approach to treat pain. You can talk about treatment with pharmacological or non-pharmacological means. The treatment with pharmacological means is mainly composed of non-opioid analgesics, opioids, tricyclic antidepressants, anticonvulsants. Morphine is the most active alkaloid and more present in opium (10%). It is the best known opioid and this is also confirmed by the fact that the analgesic power of analgesic drugs is almost always compared to the one of morphine. Even today, morphine is considered the most used analgesic drug to treat severe pain, such as cancer, postoperative pain and neuropathic pain. In both acute and chronic pain, the current drug therapy has considerable limitations due to the partial effectiveness of pain killers or significant side effects resulting from the use of such drugs. In addition, there are also patients who no longer respond to the drug treatment. These limitations are particularly evident in severe conditions such as drug treatment of cancer pain or postoperative pain. There is also a considerable inter-individual variability in the same perception of pain linked to genetic, environmental and socio-cultural factors. Given all these problems, the search for new therapies that limit the complications while keeping the therapeutic qualities is still going on. In this regard, in recent years, many researchers have focused their interest on the possible role of SIRT 1 in pain, showing that not only SIRT 1 but also AMPK and mTOR (two closely related proteins of SIRT 1) could be involved in the modulation of neuropathic pain and that an increase in the activity of SIRT 1 could be a promising therapeutic strategy for the prevention or treatment of neuropathic pain. In addition, recent studies suggest a possible involvement of SIRT 1 in chronic morphine tolerance. The importance of SIRT 1 in pain could be amplified by its own inhibitory action of mTOR described in many works as the main cause of addiction to morphine and the hypersensitivity to pain.
PURPOSE: Although many studies suggest an important role of SIRT 1 in pain and in pain therapy, further studies are required to confirm these data or to clarify the specific target by which SIRT 1 interacts in this context. Based on these premises, this experimental project has as general objective to evaluate the effect of activator substances of SIRT 1 on in vitro models of pain. In particular, it will evaluate the effect of both morphine and SIRT 1 activators after a single or associated administration.
MATERIALS AND METHODS: The cell lines used are human neurons neuroblastoma, SH-SY5Y. Cells were treated with morphine (1μM; 5μM) and two activators of SIRT 1, ferulic acid (25μM; 50μM) and tyrosol (10μM; 20μM), used as individually or together. Incubation times were 3 and 6 h. Levels of expression of SIRT 1 and the activation/ inhibition of AMPK and mTOR were assessed by the Western Blot technique.
RESULTS: The results of our experiments show that the expression of SIRT 1 is significantly increased in all experimental groups compared to the control group, both when
the cells treated with morphine and when are treated with ferulic acid and tyrosol. The analysis of the results of the associations among the substances (activators of SIRT 1 + morphine) highlighted how their associated administration creates an increased expression of SIRT 1 than the one obtained by adding the effects of the single administrations. In addition, all three substances cause an activation of AMPK and an mTOR inhibition when administered individually.
CONCLUSION: The results of the present work show that all the tested substances are not only able to activate SIRT 1 but are also able to activate AMPK and inhibit mTOR. The importance of these data is confirmed by the role of these proteins in the pathway of pain confirming, finally the role of SIRT 1 as a critical structure in the nociceptive pathway and the importance of increased activity of SIRT 1 as a promising therapeutic strategy for the prevention or treatment of acute or persistent pain.
