Da dicembre 2019 la comunità scientifica sta lavorando velocemente per trovare una cura efficace per l’epidemia di COVID-19 provocata da Sars-CoV-2. Tale infezione porta ad un quadro sintomatologico variabile da un punto di vista respiratorio e per questo motivo non è stato attualmente individuato un trattamento efficace per tutti i pazienti colpiti.
In un anno e mezzo sono state sperimentate varie terapie farmacologiche, tra cui il tocilizumab che hanno portato a risultati positivi. Questo farmaco, comunemente utilizzato per il trattamento delle patologie reumatologiche, è un anticorpo monoclonale inibitore del recettore dell’interleuchina 6.
Lo scopo di questa tesi è quello di analizzare le linee guida esistenti per l’utilizzo del tocilizumab nei pazienti ospedalizzati affetti da COVID-19. Gli studi che hanno maggior contribuito alla creazione delle linee guida sono stati RECOVERY e REMAP-CAP.
I risultati ottenuti mostrano come i benefici maggiori dati dalla somministrazione del farmaco sono presenti nei pazienti che assumono la terapia entro 24 ore dall’inizio del supporto ventilatorio sia invasivo che non invasivo e dopo l’assunzione di corticosteroidi sistemici. In attesa di ulteriori studi, tali indicazioni rimangono le uniche per l’utilizzo del tocilizumab nel trattamento del COVID-19.
Since December 2019, the scientific community has been working fast to find an effective cure for the COVID-19 epidemic caused by Sars-CoV-2. This infection leads to a variable impact of respiratory symptoms and for this reason an effective treatment for all affected patients has not been identified currently.
In a year and a half, various drug therapies have been tested, including tocilizumab which have led to positive results. This drug, commonly used for the treatment of rheumatological diseases, is a monoclonal antibody that inhibits the interleukin 6 receptor.
The purpose of this thesis is to analyze the existing guidelines for the use of tocilizumab in hospitalized patients impacted by COVID-19. The studies that contributed most to the creation of the guidelines were RECOVERY and REMAP-CAP.
The results obtained show that the greatest benefits are given by the administration of the drug within the first 24 hours from start of ventilarory support (invasive and non-invasive) and after taking systemic corticosteroids. Pending further studies, these indications remain the only ones for the use of tocilizumab in the treatment of COVID-19.
