In Medicina Veterinaria sono presenti pochi studi che trattano dell’aspetto ecografico dello stomaco nei cani con azotemia, nonostante il coinvolgimento gastroenterico sia frequente. Il nostro studio retrospettivo ha l’obiettivo di descrivere, da un punto di vista ecografico, la gastropatia in corso di insufficienza renale acuta o acuta su cronica, affrontandola anche in relazione a parametri clinici. A questo scopo, sono stati revisionati le immagini, i video ecografici e le schede cliniche di 113 cani, di età e razze diverse, giunti all’Ospedale Didattico Veterinario “Mario Modenato” del Dipartimento di Scienze Veterinarie dell’Università di Pisa tra Giugno 2010 e Marzo 2024. I soggetti inclusi mostravano sintomatologia gastroenterica e sistemica, valori di creatinina e BUN superiori ai range di riferimento e che sono stati sottoposti ad un’ecografia addominale completa entro sette giorni dagli esami biochimici. Il sintomo più frequente è stato il vomito. Le evidenze ecografiche riscontrate a carico dello stomaco riguardavano maggiormente la parete, la tonaca mucosa e sottomucosa. Tutti i soggetti avevano un ispessimento della parete. Il 13% dei casi aveva solo l’interessamento parietale, con un ispessimento diffuso o a carico di un singolo strato; l’87% anche modificazioni a livello della mucosa, come l’aumentata ecogenicità e la presenza di una stria o di una banda iperecogena, compatibile con calcificazione, della sottomucosa, come la diminuita ecogenicità, riferibile a edema parietale, o di entrambe. La banda iperecogena è stata l’alterazione mucosale più ripetuta. Tra le lesioni gastriche non sono state osservate erosioni o ulcere della parete. La mineralizzazione delle pliche gastriche si vedeva all’esame radiografico in otto cani. Per un solo soggetto l’esame istologico della parete gastrica era disponibile per la consultazione. In conclusione, il nostro studio ha fornito una rappresentazione varia dell’aspetto ecografico della gastropatia in corso di insufficienza renale acuta, mettendo a disposizione una valutazione più recente della malattia.
In Veterinary Medicine there are few studies that discuss the ultrasonographic aspect of the stomach in dogs with azotemia, despite gastrointestinal complications are common. The aim of our retrospective study was to describe, from the sonographic point of view, the gastropathy that occurs in acute or in acute-on-chronic kidney injury, in relation also to clinical parameters. For this purpose, sonographic images, videos and clinical reports of 113 dogs, of different ages and breeds, showed up at the “Mario Modenato” Veterinary Teaching Hospital of the University of Pisa between June 2010 and March 2024, were reviewed. Dogs included had systemic and gastro-enteric symptoms, creatinine and BUN values over the reference ranges and a total abdominal ultrasound exam in the seven days after the biochemical tests. Most common clinical sign was vomiting. Sonographic evidences in the stomach involved mostly the wall, the mucosa and the submucosa. All patients had thickening of the gastric wall. 13% had only the involvement of the wall, with a thickened wall, widely or concerning a single layer; 87% also had involvement of the mucosal layer, with increasing of its echogenicity and the presence of a hyperechoic streak or line, referable to calcification, of the submucosal layer, with decreasing of its echogenicity, referable to wall edema, or even both. The hyperechoic line was the most frequent change. Gastric erosions or ulcers weren’t observed. The mineralization of gastric folds was seen even in radiological images in eight dogs. On one occasion, the histological examination was available for reviewing. In conclusion, our study offers a various presentation of the sonographic features of the gastropathy in acute kidney injury, providing a most recent evaluation of the disease.
