This study evaluated the feasibility and safety of granulocytapheresis (GMA) in inducing and maintaining remission in Crohn's disease and Colitis Ulcerative. Corticosteroids are a mainstay of acute therapy for moderate to severe Ulcerative Colitis or Crohn’s disease, however up to 40% of patients do not respond to high-dose steroid therapy: steroid-resistant patients, a similar portion of patiens require increasing dose of steroids, to maintain remission, with a risk of developing adverse avents, steroid-dependent patients. New therapeutic approaches have amounting importance the clinical outcame in patients with active steroid-resistant or steroid-dependance IBDS. In particular, some trials have indicatet that granulocytapheresis, a technique that selectively sequestrates granulocyte and monocyte subpopulations, can be a useful and safe option to induce clinical remission in patients with IBD. We report the results of a prospective observational experience on the use of GMA conducted at the Pisa University Hospital. Included patients were treated with five sessions (1 session/wk) of GMA (Adacolum TM) and were evaluated at the end of five-week course at 6 months and at 12 months. The activity of the disease was evaluated by CDAI for Crohn's Disease and CAI for Ulcerative Colitis, we also analysed the course of laboratory parameters and performed safety evaluations.
