Il glioblastoma (GB) rappresenta il tumore maligno primitivo più frequentemente diagnosticato nel Sistema Nervoso Centrale.
Le sue principali caratteristiche sono: insorgenza in sedi di difficile approccio chirurgico, crescita rapida ed elevati tassi di recidiva. Microscopicamente presenta marcato pleomorfismo, ampie aree di necrosi, spiccata neoangiogenesi ed elevato indice proliferativo.
Nonostante i progressi in campo diagnostico e terapeutico, la sua prognosi rimane infausta.
Lo studio del microambiente tumorale (….) conferma la complessità delle interazioni tra cellule tumorali, tessuto peritumorale sano e cellule del sistema immunitario, attraverso l’attività di citochine, fattori solubili e recettori di membrana.
In particolare, il TME del GB si caratterizza per una spiccata capacità immunosoppressiva che potrebbe essere alla base dell’aggressività di questa neoplasia e della sua resistenza ai trattamenti radio- e chemioterapici.
Il presente lavoro affronta, attraverso l’utilizzo di alcuni tipici immuno-marcatori dell’immunità, i complessi meccanismi dell’immunomodulazione del TME nel GB.
Glioblastoma is the most frequently diagnosed primary malignancy in the central nervous system.
Its key features are: occurrance in areas of difficult surgical approach, rapid growth and high rates of relapse. Microscopically it shows remarkable pleomorphism, diffuse necrosis, rich neoangiogenesis and high proliferation index.
Despite progress in the diagnostic and therapeutic field, its prognosis remains not favourable.
The study of tumor microenvironment (TME) confirms how complex interaction is among tumoral cells, perilesional healthy tissue and immune system cells, through cytokines, soluble factors and membrane receptors.
Specifically, GB’s TME is characterized by a strong immunosuppressive ability which could explain its aggressive behaviour and its resistence to radio- and chemotherapy.
The present work confronts, using some of the typical markers of immunity, the complex mechanism of immunomodulation in GB’s TME.
