Summary
La fisiopatologia dell'emicrania non è completamente compresa, l'evidenza suggerisce che i fattori dietetici svolgono un ruolo in diversi possibili meccanismi. La dieta può avere un effetto sulla modulazione di neuropeptidi, neurorecettori e canali ionici, sistema nervoso simpatico e metabolismo cerebrale del glucosio, causando infiammazione, rilascio di ossido nitrico e vasodilatazione. Sono stati identificati diversi fattori presenti nell’alimentazione che possono scatenare l'emicrania:i fattori scatenanti più comunemente riportati spesso includono cioccolato, formaggio, noci, agrumi, carni trasformate, glutammato monosodico, aspartame, cibi grassi, caffè e alcool. Il digiuno e l'alcol sono fattori scatenanti nel 44% e nel 27% delle persone con emicrania, rispettivamente. Durante il mese di Ramadan, c'è stato un aumento del numero medio di giorni di emicrania tra le persone osservanti rispetto al mese (di controllo) seguente. Ecco perchè diete di eliminazione prendono sempre più campo nella terapia dell'emicrania accanto a questo molti nutraceutici tra cui Partenio, melatonina,butterpur,magnesio, vitamina C e D, il complesso delle vitamine B. Terapie comportamentali sono di supporto e utili in terapia da affiancare alle cure tradizionali: come tecniche di rilassamento quali yoga o l'agopuntura fino al trattamento con ipnosi.Importante è anche il ruolo del microbiota nella patogenesi dell'emicrania con cui condivide mediatori infiammatori comuni e tuttora oggetto di studio.
Migraine physiopathology is not fully understood, evidence suggests that dietary factors play a role in several possible mechanisms. The diet can have an effect on the modulation of neuropeptides, neuroreceptors and ion channels, sympathetic nervous system and brain glucose metabolism, causing inflammation, release of nitric oxide and vasodilation. Several factors present in nutrition that can trigger migraines have been identified: the most commonly reported triggers often include chocolate, cheese, nuts, citrus fruits, processed meats, monosodium glutamate, aspartame, fatty foods, coffee and alcohol. Fasting and alcohol are triggers in 44% and 27% of people with migraines, respectively. During the month of Ramadan, there was an increase in the average number of migraine days among observant people compared to the following (control) month. That's why elimination diets are increasingly taking place in migraine therapy alongside this many nutraceuticals including Partenium, melatonin, butterpur, magnesium, vitamin C and D, the complex of vitamins B. Behavioral therapies are supportive and useful in therapy to be combined with traditional care: as relaxation techniques such as yoga or acupuncture up to treatment with hypnosis. Also important is the role of the microbiota in the pathogenea of migraine with which it shares common inflammatory mediators and still being studied.
