Riassunto
The activities of Health at Home project (H@H) are settled to the development of a new and efficient system for home assistance, addressed to all those elderly persons suffering from Chronic Heart Failure (CHF).
Such a chronic pathology has been proven to be the most common reason for aged people hospitalisation, and new cases are recorded every year (around 3.6 millions in Europe) together with an increasing request for treatments and medical cares.
And the scenario is designed to get more and more complicated from both patients and hospitals point of view. The former will witness and feel the effect of the increased number of CHF sufferers, and hence the risk of a worse assistance, while the latter will record raised costs for broader and overcrowded hospitalisation, disease management and therefore additional resources, not to mention all linked drawbacks .
The development of a home monitoring system is well situated into this framework, aimed at reducing patients relapses, minimizing their uneasiness and discomfort and, at the same time, enhancing the quality of their lives thanks to the chance to securely proceed with ordinary life.
Indeed, patients will be asked to periodically wear some sensor-modules for home measurement of those physiological parameters identified by clinicians as the most significant, which will be sent to health organizations for medical supervising and analysis. Patient safety will be guaranteed by an automated alerting system which will immediately warn the same health organizations against the onset of a critical situation.
