Negli ultimi vent’anni l’incidenza delle infezioni fungine, causate da funghi patogeni ed
opportunisti, è significativamente aumentata parallelamente a un elevato tasso di morbilità e
mortalità della popolazione e dei pazienti a rischio. Le cause sono inerenti ai progressi della
medicina, all’infezione da HIV, alle nuove tecniche chirurgiche invasive, ai trapianti di organo ed
all’uso massivo di antibiotici antibatterici. È aumentata sempre di più, quindi, la necessità di
ottenere risultati celeri e specifici sugli agenti eziologici che scatenano tali infezioni in modo tale
che i clinici possano subito agire sui paziente con un’appropriata terapia antifungina.
I sistemi diagnostici, attualmente disponibili nei laboratori, richiedono spesso lunghi tempi di
esecuzione e non sempre permettono, soprattutto per le specie di recente definizione, una accurata
discriminazione. In tale ottica nel presente lavoro sono state messe a punto nuove procedure
diagnostiche per l’isolamento ed identificazione di ifomiceti di interesse clinico.
Le nuove procedure diagnostiche sono state messe a punto utilizzando nuovi terreni di coltura per la
crescita degli ifomiceti, nuove e diverse modalità di incubazione dei terreni di coltura tra cui il
sistema semiautomatizzato HB&L (Alifax,Italia) e nuovi metodi di identificazione degli ifomiceti
ovvero la spettrometria di massa con tecnologia MALDI-TOF (Matrix Assisted Laser Desorption
Ionizzation-Time Of Flight) della Bruker Daltonics .
Dai risultati ottenuti si evince che il MALDI TOF, utilizzato con una libreria aggiornata come
“Filamentous Fungi Library 1.0” in associazione con sabouraud liquido, ad esempio HB&L
Sabouraud Kit (Alifax), e con sistemi semiautomatizzati di incubazione come lo strumento HB&L
(Alifax, Italia), fornisce, in tempi notevolmente ridotti, validi risultati sia sull’identificazione di
specie sia sui tempi di refertazione.


In the last twenty years the incidence of fungal infections caused by opportunistic fungal pathogens,
has adavanced significantly, at the same time, a high rate of morbidity and mortality directly
related to development of the in the population at risk.
The causes concerning the medicine progress, HIV infection, new invasive surgical techniques,
organ transplants and the massive use of antibacterial antibiotics.
It is increased more and more, than, the request to obtain fast specific solution on the etiologic
agents that caused this infections so that the clinician can act well on patient with appropriate/ right
antifungal therapy.
Diagnostic systems still has required long times of performances and are not always allow an
accurate discrimination above all for species of recent definition,. In this perspective, the present
work have been developed new diagnostic procedures for the isolation and identification of of
clinic filamentous fungi al interest.
The new diagnostic procedures have been developed using new culture media for the growth of
filamentous fungi ), new and different ways of incubation of culture media including the system
semi-automated HB&L (Alifax, Italy) and new methods for identification of filamentous fungi or
mass spectrometry with MALDI TOF (Matrix Assisted Laser Desorption Ionizzation-Time Of
Flight) of the Bruker Daltonics.
From the results obtained with the MALDI-TOF method we know that this method used with a
current library come “ Filamentous Fungi Library 1.0”, linked with sabouraud liquid, for example
HB&L kit (Alifax) and with semiautomatic system of incubation like a HB&L kit (Alifax)
instrument, supply, in a few time, efficient result as the identification of species as on time of
dismissal.
