Objective - To show that there are differences in the distribution of alterations of Idiopathic Pulmonary Fibrosis (IPF) (honeycombing , and reticulation) in the zones of the lung and differences about evolution of the disease in terms of progression to follow up and mortality, in relation to smoke and therapy whit Pirfenidone.
Materials and Methods - We conducted a retrospective study in two groups of patients of 56 and 84 patients respectivlely, with diagnosis of IPF; both groups are divided in non-smokers and ex-smokers (<20 pack-years and > 20 pack-years)
The distribution and the progression of the abnormalities and the correlation with mortality rate were assessed using Kruskal-Wallis and two-tailed Mann–Whitney tests, linear regression analysis and Spearman’s correlation analysis, log-rank test and the the Kaplan-Meier method.
Results - A significant increment of fibrotic score and honeycombing rate was demonstrated in smokers.
However has not been demonstrated significant difference in mortality between smokers and non-smokers and in relation of therapy.
Conclusion - In our study smoke promotes the progression of the disease.
